Our duties

We are required by law to keep your protected health information (PHI) private, to give you this notice of our legal duties and privacy practices, to follow the terms of the notice currently in effect, and to notify you if a breach of your unsecured PHI occurs.

• Payment — to bill and collect payment from Medicaid, your MLTC plan, other insurers or you.

• Health care operations — for quality review, staff training and supervision, compliance audits, licensing and accreditation.

• Appointment reminders and information about services related to your care.

• As required by law, and for public health activities, abuse or neglect reporting, health oversight agencies (including the New York State Department of Health), judicial and administrative proceedings, law enforcement, coroners and funeral directors, organ donation, research under approved protocols, to avert a serious threat to health or safety, specialized government functions, and workers' compensation.

• To business associates who perform services for us (for example, electronic visit verification and scheduling software vendors, billing services) under written agreements requiring them to protect your information.

Uses and disclosures that require your written authorization

Most uses and disclosures of psychotherapy notes, marketing, and any sale of PHI require your written authorization. Other uses not described in this notice will be made only with your written authorization, which you may revoke in writing at any time.
